Get ready to Fly – Parental Consent and Waiver Form
Get ready to fly by filling out and returning a Parental Consent Form. If you want to go flying you must have the consent form signed by a parent/guardian. The RV12 is still several months from flying but having a signed form will allow you to fly in one of the other aircraft that are available.
